Pursuant to the resolution dated 1404/02/15 of the Board of Directors and Inspectors, and in accordance with Chapter 2 of Article 12 of the Articles of Association and its notes:
I hereby request membership in the Production & Distribution Cooperative of the Employees and Retirees of the Ministry of Industry, Mine and Trade (Shetacob) and commit to the following:
1- I am not a member of a similar cooperative. If proven otherwise, Shetacob Cooperative shall be entitled to unilaterally terminate my membership.
2- Should my employment with the Ministry of Industry, Mine and Trade or its affiliates end (except for retirees), I shall notify the management of Shetacob Cooperative in writing and request termination of my membership. Failing that, the Cooperative shall be entitled to unilaterally end my membership as of the employment end date under the Articles of Association, and I shall have no right to object.
